REQUIREMENTS

Home isolation is recommended by M.P. Shah Hospital ONLY if you meet the criteria below:

I have a laboratory confirmed diagnosis of COVID-19
I am asymptomatic or have mild symptoms of COVID-19 (Mild disease is characterized by the absence of evidence of viral pneumonia or hypoxia-reduced level of oxygen in your blood)
I do not have any co-morbidity (any chronic medical condition that you have been diagnosed with prior to or concurrently with your COVID-19 diagnosis)
I have reviewed the checklist for assessment of environmental conditions for home based access to a suitable space for home-based care https://www.health.go.ke/wp-content/uploads/2020/06/Home-Based-Isolation.pdf

In addition to the criteria above you need to agree to the following:

I have provided the information above to the best of my knowledge and having understood the home-based isolation and care guidelines from the Ministry Of Health https://www.health.go.ke/wp-content/uploads/2020/06/Home-Based-Isolation.pdf.
MP Shah Hospital is not liable for reviewing my preferred site for home based isolation and care and are providing the package based on my or a county public health officer’s assessment of my preferred site.
MP Shah Hospital is not liable for any treatment that I may take that has not been prescribed by a medical practitioner assigned to my telemedicine consult.
MP Shah Hospital is not liable for any complications that may arise due to the omission of any vital information on my part during the course of my home-based care.
MP Shah Hospital will not be responsible for any additional medical review or treatment except those mentioned in the package.
MP Shah Hospital is not liable for any incident related to equipment that will be required the checking my temperature, heart rate and pulse oximetry.
MP Shah Hospital may provide services but is not responsible for my evacuation and inpatient treatment if my condition changes and necessitates a higher level of treatment.
